Home Care after a Hospital Stay
Home care will help to reduce the possibility of readmissions after a hospital stay. We are available for setting up support on short notice, and will coordinate with your hospital discharge planner.
Assistance with Daily Activities
Helping clients with everyday tasks to promote independence and a better quality of life. Our services include bathing, dressing, meal preparation, light housekeeping, medication reminders, compionship and other daily activities needed to meet your needs.
Support for Title 19 Medicaid Clients
We are a certified provider under the Connecticut Home Care Program for Elders and the Personal Care Assistance (PCA) Waiver program. We work with SWCAA case managers to coordinate care for seniors who have this benefit.
